Information about the Sports Physicals.
Sports Physicals
- For Middle School or High School Athletes in Nebraska.
- Walk Ins Welcome, but we recommend calling Fremont Therapy & Wellness to set a timeslot at 402-512-3893.
- To save time we encourage downloading the Sports Physical Form and filling out ONLY THE HISTORY FORM of the physical prior to arrival.
- If the athlete has a disability, they will need to fill out the SUPPLEMENT TO ATHLETE HISTORY PAGE.
- Sports physicals are completed by 1 of 2 Nurse Practitioners & follow the NSAA form.
- Student Athletes are encouraged to wear sports and females to wear shorts and sport-bras.
- Student Athletes will be progressed through different stations of assessments and should take around 20 min to complete.
- Cost is $20 and must be paid at time of service by cash, check or credit card to Fremont Therapy & Wellness.
- Student Athletes will be given a copy of their form and a copy will be kept in our system.
